FRANÇAIS : La place du cannabis dans la pharmacopée occidentale a varié au cours du temps. La découverte du principal composé actif, le tétrahydrocannabinol (THC), et d'un système cannabinoïde endogène a suscité un regain d'intérêt pour cette substance. La toxicité aiguë du cannabis est faible mais l'inhalation chronique de fumée induit des modifications bronchiques cliniques et histopathologiques indésirables. Les études cliniques ont été effectuées surtout avec le THC sous forme orale. Elles ont porté notamment sur l'effet antalgique, la stimulation de l'appétit lors de cancer et d'infection VIH et le traitement des vomissements induits par la chimiothérapie. Alors que le THC montre une certaine efficacité dans les indications, la fréquence des effets indésirables, principalement sur le système nerveux central, restreint actuellemnet son utilité thérapeutique. (Résumé d'auteur). ENGLISH : The role of cannabis in the Western pharmacopoeia varied in the course of time. The discovery of the main psychoactive component, tetrahydrocannabinol (THC), and of the endogenous cannabinoid system awakened a renewed interest in that substance. The acute toxicity of cannabis is low but chronic smoking causes adverse clinical and histopathological alterations in the respiratory airways. The oral form of THC was used in most clinical research. The analgesic effect, the stimulation of appetite in cancer and AIDS patients and the antiemetic properties in chemotherapy-induced nausea and vomiting were assessed in clinical trials. THC demonstrates some efficacy in these indications. However the incidence of adverse effects, mainly on the central nervous system, limits its therapeutic use. (Authors'abstract). |
