FRANÇAIS : Dans cet article, différentes hypothèses concernant les soubassements psychiques des addictions doubles - alcool et toxicomanie - sont étudiées. À l'origine de la toxicomanie, on trouve souvent un traumatisme lié à une expérience réelle mais incommunicable, nommé crypte en raison de .son caractère secret : la drogue vient compenser le déficit affectif créé par le clivage dans le Moi. Les traces d'un trauma subi par les ascendants - générant un « travail de fantôme » - sont également fréquentes : elles provoquent un sentiment de vide ou d'étrangeté que la toxicomanie permet d'oblitérer. L'alcoolisation est aussi généralement associée à un clivage du Moi - « crypte » ou, en l'absence de secret, « incorporat » (Tisseron, l990) - et comble littéralement le manque à soi-même. Ces éléments ne suffisent donc pas à différencier, au niveau des fondements psychiques ou des effets recherchés, alcoolisme et toxicomanie. Les cas d'Élodie et d'Aurélie, héroïnomanes, autorisent une interprétation nouvelle. Ces patientes n'utilisent pas l'alcool de façon addictive, mais pour s'identifier à la souffrance d'un parent alcoolique, dans le double but de comprendre sa dépendance et de s'en dégager : être reconnu soi-même comme souffrant. L'auteur insiste en conclusion sur la nécessité d'un diagnostic différentiel entre consommations de drogue et d'alcool, qui puisse déterminer utilement l'orientation des patients. (Résumé de la revue) ENGLISH : In this paper, several hypothesis about the psychic origins of double addictions - alcohol and heroin - were studied. We frequently find as a ground of drug addiction a hidden trauma, in relation with a real experience never communicated. This trauma is called « crypt », because of its secrecy: drug is used in order to compensate the affective deficiency created by the splitting of ego. The traces of a trauma endured by ancestry - generating a psychic « ghost-work » - are also frequent : they provoke a sensation of emptiness or of strangeness that drug addiction tries to obliterate. In the same way alcoholism is also generaly associated with the splitting of the ego - « crypt » or, in the absence of secret, minor splitting of the ego (called « incorporat » by Tisseron) - and gratifies the lack of inner communication. Those elements are not enough to differentiate, at the levels of psychic foundments and sought effects, alcoholism and drug addiction. The cases of Elodie and Aurelie, joung women addicted to heroin, lead to a new interpretation. This patients don't use alcohol in an addictive manner, but in order to sympathize the suffering of an alcoholic parent, in the double aim to understand his dependance to alcohol and to get oneself free of it : being recognized as a suffering person. The author insists in his conclusion upon the necessity to reach a differential diagnosis between drug uses and alcohol uses, which can determine usefully the orientation of patients.
