Buts : examiner les différences de comorbidité psychiatrique entre les groupes d'héroïnomanes hospitalisés et les groupes d'héroïnomanes incarcérés à Taiwan. Type d'étude : les auteurs ont examiné la prévalence des troubles psychiatriques coexistants tels que répertoriés dans le DSM-III-R, incluant les troubles de la personnalité. Lieux : un hôpital psychiatrique et deux prisons. Participants : 260 héroïnomanes incarcérés, et 47 usagers d'héroïne ayant volontairement demandé de l'aide dans un hôpital psychiatrique, ont été interrogés par des psychiatres confirmés. Mesures : la comorbidité psychiatrique a été évaluée à l'aide de deux instruments psychométriques, le Psychiatric Diagnostic Assessment (PDA) et le Structured Interview for DSM-III-R Personality Disorders (SIPD-R). Résultats : le taux de troubles psychiatriques coexistants chez les héroïnomanes était différent selon le lieu: 83% des sujets hospitalisés et 66% des sujets incarcérés avaient un diagnostic d'au moins un trouble coexistant (axe 1 ou II). Les troubles coexistants les plus fréquents tels que définis par l'axe I du DSM-III-R étaient les troubles liés à l'usage d'une autre substance (alcool ou amphétamines), tandis que pour l'axe II, on retrouvait davantage le trouble de la personnalité antisociale. Life-time prevalence of DSM-III-R-based coexisting psychiatric disorders, including personality disorders, were surveyed. Settings. A psychiatric hospital and two prisons. Participants. Two hundred and sixty heroin users who were incarcerated in prisons, and 47 heroin users who voluntarily sought help in a psychiatric hospital were interviewed by board-certified psychiatrists. Measurements. Using two psychometric instruments, the Psychiatric Diagnostic Assessment (PDA) and the Structured Interview for DSM-III-R Personality Disorders (SIPD-R), psychiatric co-morbidity was assessed. Findings. Different life-time rates of coexisting psychiatric disorders amnong heroin addicts in different settings were found: 83% of hospital sub cis and 66% of incarcerated subjects were diagnosed as je having at least one coexisting axis I or II disorder.
