FRANÇAIS : Les changements d'organisation des services de soins aux toxicomanes dans le nord-ouest de l'Angleterre dans les années 80 ont offert une occasion unique d'étudier un groupe de toxicomanes aux opiacés qui avaient quitté leur traitement. Cet article se rapporte à 60 toxicomanes qui étaient abstinents lors du suivi (2-3 années après leur entrée en traitement). Pour la majorité des sujets, les traitements reçus dans l'intervalle l'ont été de la part de non-spécialistes (24 médecins généralistes et 32 en hôpital général). Les sujets avaient aussi reçu l'aide d'agences non statutaires (35). D'autres formes d'aide ont été trouvées auprès d'amis, de membres de la famille et de bénévoles. L'acceptation sociale, les problèmes législatifs, des difficultés financières et des séjours en prison ont été fréquemment cités comme facteurs ayant favorisé l'abstinence. Lors du suivi, des améliorations ont été rapportées dans les relations personnelles et familiales, même s'il y a eu moins de progrès dans la relation à l'emploi, à l'argent et à la maison. Une implication plus importante par rapport au système judiciaire a été significativement associée à une plus grande fréquentation des services de soins au moment du suivi. Cet article présente les chemins d'accès à l'abstinence, à travers ou hors un traitement, pour ce groupe de toxicomanes. Il pointe aussi l'importance d'inclure des échantillons hors traitement dans les évaluations des interventions de soins. ENGLISH : Structural changes in the organization of drug treatment services in the northwest of England during the early 80s provided a unique opportunity to study a group of opiate addicts who were turned away from treatment. This paper reports on 60 opiate addicts who were abstinent at follow-up (2-3 years after their original referral). For the majority of subjects, any treatment received in the intervening period had been from a range of local, non-specialist treatment providers, including: general practices (n=24) and local hospitals (n=32). The subjects had also received assistance from non-statutory agencies (n=35). Informal sources of support were drawn upon in the form of friends, family, and voluntary agencies. Social acceptance, legal problems, financial difficulties, and imprisonment were frequently cited as motivational factors that led to abstinence. At follow-up, improvements were reported in personal relationships and family circumstances, although there was less progress in relation to employment, finances, and housing. Increased involvement with the criminal justice system was significantly associated with a greater use of treatment services at follow-up. This paper presents the treatment and non-treatment pathways that led to abstinence amongst this group of opiate addicts. It also points to the importance of including non-treatment samples in evaluations of treatment interventions. (Author's abstract.) |
