FRANÇAIS : C. B. Eap écrit à l'éditeur de European Addiction Research à propos de l'article de Wolf et Strang, paru dans cette revue sur le contrôle du dosage de la méthadone dans le sang. Il fait remarquer que l'observance du traitement devrait être basée sur le postulat suivant : "quand l'observance est bonne, la relation entre la concentration dans le plasma et la dose de méthadone est fortement corrélée", un postulat qui n'est pas soutenu par les données publiées sur la pharmacocinétique de la méthadone. Il y a une grande variabilité individuelle dans les activités enzymatiques, liée à des causes génétiques et environnementales. Reconnaître que cela interagit sur les concentrations plasmatiques est de la plus haute importance. C'est un argument en faveur de la nécessité d'individualiser les traitements ce qui est loin de se faire malgré les demandes de nombreux cliniciens. ENGLISH : In a recent paper by Wolf and Strang, the authors discussed in details the possible advantages of therapeutic drug monitoring (TDM) of methadone and stated in conclusion their anticipation that 'TDM of methadone (in the blood) will become a standard investigative tool in the methadone programme of tomorrow', an opinion that I share. One of the advantages of TDM is that it can be useful to assess compliance, reducing thus the important problem of methadone diversion. However, controlling compliance would be based on the assumption that 'when compliance is good, the relationship between plasma concentration and methadone dose is highly correlated', an assumption which is not supported by published data on the pharmacokinetics on methadone. Recognizing the problems of the interindividual variability of methadone pharmacokinetics and concentrations is of major importance. It is a strong argument in favor of the need to individualize methadone treatment. which is asked by many clinicians and is still far from being realized. in particular in the centers with arbitrarily fixed dose policy. I hope, and believe, that TDM of methadone will t ultimately help to better consider each patient's individual need, and will become, as stated by Wolf and Strang 'a standard investigative tool in the methadone program of tomorrow'.
