FRANÇAIS : L'expérience sur neuf ans de fonctionnement d'un programme public, multidisciplinaire de soin à domicile pour des toxicomanes malades du Sida, est présentée. Depuis sa restructuration en 1993, 329 dépendants aux opiacés ont été pris en charge (86% UDVP). Leurs caractéristiques socio-démographiques sont celles propres à la population marginale. Leur moyenne d'âge est de 33.8 ans et les hommes (70.2%), ont de façon significative deux ans de plus que les femmes. Dans 37.8% des cas, la tuberculose est la maladie qui a permis de diagnostiquer le sida. 67.4% ont été classés selon les critères du CDC, dans le groupe C3 et 62.2% ont un Karnofsky index £ 70. La durée moyenne dans le programme est de 11.4 mois, en observant des différences significatives entre les sexes en faveur des femmes (plus 3 mois). Il y a eu 216 interruptions de traitement, 114 par sortie du programme et seulement 39 par perte de contact. Le contrôle de la toxicomanie, principalement par un traitement de maintenance par la méthadone, permet un meilleur déroulement des autres interventions du programme. La diminution de la mortalité de 1996 à 1998 est directement liée aux nouvelles thérapies antirétrovirales. ENGLISH : We present the experience of a public, multidisciplinary and integrate home care program for AIDS and drugdependent patients during nine years. Since the remodelling in 1963, we have treated 329 patients (86% intravenous users) whose social and demographic living conditions relate with a marginal group of population. Their average age was of 35.8 years and men (70.2%) were significantly 2 years older than women. In the 37.8% of the cases, tuberculosis was the main illness for diagnosing AIDS. Following the CDC criteria, the 67.4% were included in the C3 group and the 62.2% had a Karnofsky index f 70. The remaining time in the program had a media duration of 11.4 months, with significant differences between sexes, women stayed 3 more months than men. They were 216 interruptions of the program, 114 exitus and only 39 patients lost contact. Conclusions: 1. Stopping the drug abuse, mainly with the help of methadone prescription, clearly improves the accomplishment of the remaining activities of the program. 2. The decrease of the mortality during the period 1996-1998, is directly related with the new anti-retroviral therapies. (Review's abstract.) |
