L'objectif de cette étude était de vérifier les points communs et les différences caractéristiques de patients ayant à la fois des troubles psychiatriques et d'abus de drogues, accueillis soit dans des services de santé mentale, soit dans des centres de soins pour toxicomanes. 129 patients ont été étudiés dont 65 soignés pour leur abus de drogue et 64 appartenant à un programme de santé mentale. Alors que les deux groupes étaient similaires quant à leur âge et à leurs caractéristiques ethniques, des différences significatives sont apparues concernant leur profil psychiatrique : le groupe en traitement pour abus de drogues présentait des diagnostics moins lourds et aucun patient schizophrène, à l'inverse de l'autre groupe dont la majorité des patients était schizophrène. D'autres différences, telles que le statut marital ou parental, certaines inaptitudes et problèmes médicaux sont apparus comme étant directement liés à ce profil diagnostic. Ces données suggèrent que les différences significatives observées chez les patients présentant un double diagnostic pourraient être en rapport avec le type de traitement qu'ils suivent. ENGLISH : The aim of this study was to assess the similarities and differences of patients with co-existing psychiatric and substance use disorders attending treatment in either a mental health setting or a substance abuse treatment setting. A total of 129 patients were assessed, includ- ing 65 individuals from the substance abuse treatment center and 64 individuals from the mental health program. Treatment records were reviewed for diagnoses and sociodemographic data. While the two groups were highly similar with regard to age and ethnicity, there were significant differences in psychiatric profile, with the substance abuse treatment group having less severe diagnoses and no patients with schizophrenia, while the mental health treatment group had a majority of patients with schizophrenia. Other differences in the two groups, such as marital and parental status, disability status, and medical problems appeared to be directly linked with the aforementioned diagnostic profile. These data suggest important differences in characteristics of patients with cornorbid disorders that appear to be dependent on the type of treatment program they attend. For the most effective management, integrated treatment programs should be aware of these differences and tailor service provision accordingly. (Editor's abstract) |
