FRANÇAIS : De cette étude d'évaluation portant sur la prévention primaire en Europe, il ressort d'abord que les structures qui mènent des actions sont principalement des écoles, qui l'ont souvent intégrée comme une composante de l'éducation générale à la santé. Les stratégies utilisées sont le plus souvent basées sur le renforcement des compétences ou l'éducation par les pairs. La prévention primaire a pour objectif de développer les capacités d'un individu à consommer des drogues légales de manière responsable et à refuser l'usage de drogues illégales. En Europe centrale l'approche de "prévention des addictions" domine, dont l'objectif est d'empêcher le développement d'une personnalité dépendante. Pour cela, il faut agir tôt, dès le jardin d'enfant. Certains concepts de prévention semblent séduisants, mais l'absence d'évaluations suffisantes ne permet pas de conclure à leur efficacité. De plus, la majorité de ces concepts ont été importés des USA, datent des années 80 et découlent de la philosophie de "Guerre générale à la drogue", qui n'est pas partagée par tous les acteurs et théoriciens de la prévention en Europe. Une évolution semble nécessaire vers des approches nouvelles, réalistes, qui tiennent compte des changements socio-culturels, adaptées aux populations à risque, et prenant en compte les besoins locaux. Enfin on peut s'interroger sur les limites actuelles entre la prévention primaire et la prévention secondaire. ENGLISH : Carrying out the international study on the evaluation of primary prevention in Europe in the context of the Cost A6 action we could identify certain trends. Concerning struc- ture primary prevention predominantly is carried out in schools (school based drug education), and often repre- sents a component of general health education. Methodo- logically the strategies most often used are skill and com- petence training: the peer-education model is also used. The message concerning the aims of the preventive effort are complex: the objective of primary prevention seems to be to develop abilities in the individual to be able to use legal drugs responsibly and to denounce the use of illegal drugs. In middle Europe the approach of "addiction prevention" is found to be the prevailing one. This approach has the objective to counteract the development of such psychic structures which might be fostering dependent personality traits and therefore contribute to the development of addictive behaviours. Such effort starts already at an early age. even in the kindergarten. Some of these prevention concepts are looking promising. Otherwise their efficiency is not sufficiently proven since only few prevention projects have been evaluated properly. It has also to be pointed out that most of the concepts in use have been imported from the USA. They are stylised therefore in accordance to the premises of the dominating "Global War on Drugs" philosophy, a view that is not fully shared by most European prevention theoreticians and practicians. Furthermore they were developed during the early and middle 80ies, just before a dramatic change in consumption pattern of psychoactive drugs took place. For the future we can postulate some important tasks. In addition to the traditionally styled primary prevention model that will be going on despite all criticism that is evolving now, we will need novel approaches, which react sensitively to changes in the sociocultural system. They should be realistic and not oriented towards abstract rather utopistic aims. They also should not cling to conceptualisations and certain interpretations concerning addiction, whose validity is not sufficiently proven yet. It can be awaited that such programs will be found important which are adapted to the needs of risk populations, identified through exact investigation of prevailing local or regional conditions. It also can be expected that the borderline between primary and secondary prevention increasingly becomes blurred. (Review's abstract.) |
