La Clinique d'Alcoologie et Toxicomanies du Service de Psychiatrie du CHU Brugmann (Université Libre de Bruxelles) commémore, en cette année 2000, 25 années d'activités et de recherches dans le domaine des thérapies cognitivo-comportementales de l'alcoolisme. Trois périodes se sont succédées au cours du temps, en ce qui concerne les concepts et les méthodes cliniques d'application. La période dynamico-comportementale (1975-1980) a permis de situer le comportement de prise d'alcool dans la psychodynamique générale du patient alcoolique. Ont été développées des stratégies personnalisées dans chaque cas, permettant l'acquisition de comportements alternatifs à la prise de boisson. L'importance théorique des comportements spécifiques de boisson a été démontrée, en tant que structure de base (et non uniquement descriptive) de l'alcoolisme. Les conditions de programmes de consommation contrôlée d'alcool après désintoxication, ont été étudiées et appliquées. La période cognitivo-comportementale (1980-1995) a permis aussi d'intégrer des aspects plus spécifiquement de l'ordre cognitivo-affectif. On a montré toute l'importance des distorsions cognitives, des comportements gouvernés par les règles, de l'inadéquation des réactions émotionnelles, tant sur le plan du vécu psychologique que sur celui des modifications physiologiques. Un processus particulier, d'ordre cognitivo-émotionnel, largement déficitaire chez le patient alcoolique - le sentiment général de bien-être - a fait l'objet de recherches et d'applications thérapeutiques particulières. La période neuro-cognitive (1995-) a permis, à l'occasion d'investigations neuro-psychophysiologiques, de mettre en évidence toute l'importance des troubles déficitaires dans les processus cognitifs sub-cliniques, en tant que facteurs associés à la rechute, avec comme corollaire thérapeutique des programmes de réhabilitation neuro-cognitive. L'ensemble de ces méthodes d'application clinique a bénéficié, en plus de l'approche cognitivo-comportementale, d'apports issus d'autres théories : psychodynamique, gestalt-thérapie, bioénergie, analyse transactionnelle, etc... Les procédures psychopharmacologiques associées ont également été très contributives, en particulier, en ce qui concerne l'homotaurinate de calcium. (A partir du résumé d'auteur) ENGLISH : This year, the Alcoholism and Drug Addiction Clinic of the Department of Psychiatry of Brugmann University Hospital (Université Libre de Bruxelles) is celebrating its 25 years of clinical practice and research in the field of cognitive and behavioural therapy for alcoholism. The history of the concepts and practical modalities of this therapy can be divided into three successive periods. During the dynamic-behavioural period (1975-1980), alcohol consumption behaviour was able to be integrated into the alcoholic patient's general psychodynamics. Based on completion of a questionnaire identifying situations in which the alcoholic patient drinks, personalized strategies adapted to each case were developed, allowing the acquisition of alternative behaviours to drinking. The theoretical importance of specific drinking behaviours was demonstrated to be a basic structure (and not exclusively descriptive) of alcoholism. The conditions of controlled alcohol consumption programmes after disintoxication were studied and applied. The cognitive-behavioural period (1980-1995) also allowed integration of more specifically cognitive and affective aspects. The importance of cognitive distortions, behaviours governed by rules, and inappropriate emotional reactions, in terms of both psychological aspects and physiological modifications, was highlighted. These developments are similar to those described by other authors, in terms of the tension-lowering process or the search for stimulation. A particular cognitivo-emo- tional process, largely deficient in alcoholic patients, a general feeling of well-being, was the subject of research and specific therapeutic applications. The neurocognitive period (1995-), based on neuropsychophysiological investigations, demonstrated the important role of subclinical cognitive deficiencies as factors associated with relapse, with, as a corollary, neurocognitive rehabilitation treatment programmes. In addition to the cognitive and behavioural approach, all of these modalities of clinical application also benefited from approaches derived from other theories: psychodynamic, gestalt therapy, bioenergy, transactional analysis, etc. Associated psychopharmacological procedures, particularly calcium homotaurinate, also made a very important contribution.
