Une enquête prospective, menée sur six mois dans les quatre unités du service des urgences au CHU de Reims, a été réalisée dans l'objectif d'envisager des modalités de prise en charge plus spécifiques aux femmes alcooliques, selon qu'il s'agit d'un problème durable ou non lié à l'alcool. L'enquête sur les femmes alcooliques a été menée à partir d'une fiche. Trois catégories de diagnostics ont.été proposées : "Intoxication éthylique aiguë", "intoxication éthylique chronique", "intoxication éthylique aiguë sur fond d'intoxication éthylique chronique". Au cours des six mois, 125 fiches ont été remplies. 73 % de ces femmes portaient le diagnostic de problème durable lié à l'alcool contre 27 % qui présentaient à leur admission une intoxication éthylique aiguë. Tout diagnostic confondu, 70 % des femmes ont bénéficié d'un entretien auprès d'un médecin psychiatre aux urgences. Les propositions de prise en charge pour les patientes présentant un problème durable lié à l'alcool étaient préférentiellement une orientation vers une hospitalisation (hôpital général ou psychiatrique). En revanche, un suivi ambulatoire auprès d'un médecin psychiatre était généralement proposé aux buveuses "aiguës". En conclusion, les patientes présentant une intoxication alcoolique aiguë, déjà admises dans le service des urgences pour un problème lié à l'alcool et/ou évoquant à l'entretien des difficultés avec l'alcool antérieures à cette admission, devraient bénéficier avant leur sortie définitive d'un entretien avec un membre de l'équipe hospitalière d'alcoologie de liaison. Pour ce faire, le service des urgences devrait disposer de la présence concomitante d'une assistante sociale et d'un membre de l'équipe hospitalière d'alcoologie rattachés au service, disponibles quotidiennement et couvrant la journée. (Résumé d'auteur) ENGLISH : A prospective, six-month survey was conducted in the four units of the emergency department in Reims university hospital in order to assess the modalities of management more specifically applicable to alcoholic women, according to whether or not the alcohol-related problem was a lasting problem. The survey in alcoholic women was conducted by means of a questionnaire. Three categories of diagnoses were proposed: "acute alcohol intoxication", "chronic alcohol intoxication", "acute alcohol intoxication in a context of chronic alcohol intoxication". Over the six-month period, 125 questionnaires were completed. 73% of these women had a diagnosis of lasting alcohol-related problem versus 27% presenting with acute alcohol intoxication on admission. Regardless of the diagnosis, 70% of women were interviewed by a psychiatrist in the emergency department. Management proposals for patients with a lasting alcohol-related problem mainly consisted of hospitalization (psychiatric or general hospital), while outpatient follow-up by a psychiatrist was generally proposed to "acute" drinkers. in conclusion, patients with acute alcohol intoxication, already admitted to the emergency department for an alcohol-related problem and/or in whom clinical interview suggests difficulties with alcohol prior to this admission, should be interviewed by a member of the Hospital Alcohology Liaison Team before being discharged. A social worker and a member of the Hospital Alcohology team should therefore be concomitantly available all day and every day in the emergency department. (Author' s abstract) |
