Dans un centre spécialisé de soins pour alcoolodépendants, nous avons étudié la motivation pour aborder le sujet du tabagisme chez des patients dépendants à la fois à l'alcool et au tabac. À la suite d'une information sur le tabagisme, nous avons interrogé les patients sur leur intérêt pour une démarche de sevrage au cours de leur hospitalisation. Nous avons suivi le passage au stade d'action, soit par recherche d'information complémentaire, soit par tentative de sevrage au cours de l'hospitalisation, en fonction de différents critères : les caractères sociodémographiques, l'histoire du tabagisme et des alcoolisations, les signes anxieux et dépressifs, et l'intérêt initialement déclaré. Parmi les 144 patients inclus, 59 % étaient intéressés par l'information prodiguée sur le tabagisme. Parmi eux, 74 % se déclaraient motivés pour rechercher une information complémentaire et 26 % pour tenter un sevrage tabagique au cours de l'hospitalisation de quatre semaines. Dix Patients, soit 6,25 % de la population incluse, ont effectivement tenté un sevrage tabagique, mais ils représentent 36,5 % de la population intéressée. Il existe un décalage temporel et qualitatif concernant les motivations pour arrêter l'alcool et le tabac, et un hiatus entre les stades de préparation et d'action. Nous avons décrit la population intéressée pour mieux la cibler au cours des sensibilisations à l'arrêt du tabac. L'interrelation des dépendances alcoolique et tabagique rend en effet particulière la prévention dans cette population fortement exposée. (Résumé d'auteur) ENGLISH : In a specialized centre for alcohol-dependent patients, we studied the motivation to approach the subject of smoking in patients dependent on both alcohol and tobacco. Following information about smoking, we asked the patients whether they would be interested in participating in a smoking cessation programme during their hospitalization. We assessed the patients' adoption of active measures in terms of requests for further information or attempted smoking cessation during hospitalization, according to various criteria: sociodemographic characteristics, history of smoking and alcohol abuse, signs of depression and anxiety, and the interest initially expressed. 59 % of the 144 patients included were interested in the information about smoking. 74 % of them declared that they were motivated to seek further information, and 26 % were motivated to attempt smoking cessation during the four-week hospitalization. Ten patients (6.25 % of the population included) effectively attempted smoking cessation, but they represented 36.5% of the population interested. There was a temporal and qualitative gap concerning motivations to stop alcohol and smoking, and a lag-time between the preparation and action phases. We have described the interested population to more precisely target this population during smoking cessation awareness campaigns. The interrelation between alcohol dependence and smoking makes prevention particularly important in this high-risk population. (Author' s abstract) |
