FRANÇAIS : Le cannabis est un psychotrope de plus en plus consommé, le plus souvent de façon clandestine, dans les pays industrialisés et dans le monde. Parmi les nombreux effets qu'il peut induire, les phénomènes de flash-back ont été reconnus comme entités cliniques seulement depuis les années 1970 et ont été peu étudiés. Le cas décrit concerne un acte de violence déclenché, selon l'analyse médico-légale qui en a été faite, par un flash-back cannabique, sans participation d'autres substances psychotropes. Fait exceptionnel, un dosage sanguin de tétrahydrocannabinol a pu être réalisé dans les minutes qui ont suivi le début du flash-back. Ce cas rappelle que la participation éventuelle de phénomènes de flash-back ne doit pas être négligée lors de la recherche des causes d'accidents ou de crimes a priori inexpliqués. (Résumé de la revue.) ENGLISH : Cannabis is a psychoactive drug more and more widely consumed in industrialized countries and in the world. Amongst the numerous effects it can induce, flashback phenoma have been scientifically recognized only since the 70's. This case regards a young man who offended a friend without any objective reason. The report of the forensic psychiatrist demontiated that the offence was committed under the influence of a cannabis flashback. The last time the offender had consommed cannabis, probably from Netherland, was two weeks before the acts. A plasmatic detection was realized and showed a level at 6 ng/mL, thirty minutes after the beginning of the flashback. This case confirmes the clinical entity of cannabis flashback and gives an exceptionnal indication on THC blood concentration at the time of the relapse. But the pharmacokinetics of the phenomenon are largely unknowed and long term studies concerning the metabolism, not only of THC but also of its metabolits, are needful. On the forensic standpoint, the possible participation of flashback phenoma must not be neglected when grounds of unexplained accidents or crimes are searched. (Review's abstract.) |
