Les médecins du travail hésitent souvent sur leurs objectifs. Pour que les objectifs soient évaluables, nous proposons de renoncer à individualiser le rôle spécifique de l'alcool dans les dommages suivants, aisés à recenser par la Direction des ressources humaines : accidents de toute nature, bagarres, recours à l'infirmerie, hospitalisations, invalidités, morts violentes, mutations demandées par le médecin du travail, licenciements individuels, absentéismes et malfaçons. Raréfier ces événements est motivant pour les partenaires. Les techniques diffèrent suivant que l'alcoolisation est connue, qu'un incident procure une occasion, que l'alcoolisation reste à dépister, et que le service est ou non interentreprises. Un incident procure une occasion pourvu que soit négocié un contrôle d'alcoolémie fait par un secouriste : facultatif mais garantissant l'absence de sanction. À défaut d'incident, plutôt qu'un dépistage centré ouvertement sur l'alcool, des mesures collectives et des outils individuels sont recommandés. Les médecins interentreprises n'ont guère d'autres ressources qu'un dépliant d'information et un questionnaire de santé. Les résultats en quatre ans, dans une entreprise métallurgique de Carignan, ont été la chute du nombre annuel des accidents du travail de 495 à 293, et la chute du taux de cotisation correspondant de 4,21 à 3,16. Les rétablissements ajoutés aux améliorations durables ont concerné 11 % du personnel. (Résumé d'auteur) ENGLISH : Occupational health physicians are often hesitant about their objectives. To define evaluable objectives, the authors propose that the specific role of alcohol should no longer be individualised in the following forms of damage, easily identified by Human Resources: all forms of accidents, fights, infirmary visits, hospitalisations, invalidity, violent deaths, job changes requested by occupational health physicians, individual dismissals, absenteeism and work defects. All social partners would be motivated by a reduction of these events. Techniques differ according to whether alcohol abuse is known, whether an incident constitutes an opportunity for detection, whether alcohol abuse remains to be detected, and whether or not the occupational health service is an intercompany structure. An incident constitutes an opportunity for detection, provided a sample for blood alcohol assay can be obtained by a first-aid worker: optional, but ensuring absence of penalty. In the absence of an incident, collective measures and individual tools are recommended rather than detection openly concentrated on alcohol. Very few resources are available to intercompany doctors apart from an information handout and a health questionnaire. The four-year results in a Carignan metal-working company revealed a fall in the annual number of work accidents from 495 to 293 and a corresponding fall in accident insurance contribution rates from 4.21 to 3.16. Recoveries combined with lasting improvements concerned 1 1 % of personnel. (Author' s abstract) |
