Si l'on considère certaines conceptions psychodynamiques développées à propos de l'autisme infantile, on ne peut s'empêcher de faire le rapprochement entre les éléments cliniques et psychopathologiques décrits par des auteurs comme Tustin et certains aspects de la pathologie alcoolique. L'auteur s'attache ainsi à mettre en lumière la dimension autistique observable dans le fonctionnement psychique de l'alcoolique, en soulignant la recherche, dans le comportement d'alcoolisation, d'une altération de la conscience et d'un effacement d'une réalité extérieure trop angoissante. Ce constat, qui réaffirme le caractère archaïque de la faille psychique de l'alcoolique, permet de comprendre plus précisément la nature de l'angoisse qu'il tente d'anesthésier par les effets de l'alcool et de réviser le statut de l'alcool dans son économie psychique. La démarche thérapeutique se doit donc de favoriser, par une mise en confiance progressive, l'expression et la mise en mots d'une souffrance jusqu'alors réduite au silence. (Résumé d'auteur) ENGLISH : Some of the psychodynamic concepts proposed to explain infantile autism present a certain similarity with the clinical and psychopathological elements described by authors such as Tustin and certain aspects of alcoholic disease. The author tries to highlight the autistic dimension that can be observed in the mental functioning of alcoholic patients, by emphasizing the search, in alcohol abuse behaviour, for an altered state of consciousness and masking of an excessively distressing external reality. This observation, which reaffirms the archaic nature of the psychological defect of alcoholics, provides a clearer understanding of the nature of the anxiety that the subject tries to anaesthetise by the effects of alcohol and helps to place the status of alcohol in the patient's mental economy. The therapeutic approach, based on the progressive development of a relationship of confidence, must therefore promote the verbal expression of a suffering previously reduced to silence.
