ENGLISH : To exarnine the current consequences of buprenorphine's international schedule and consequence of possible change, the situation in France is of interest. In France buprenorphine is accessible for the treatment of opiate dependent individuals with the least regulatory control possible in relation to current international regulations. Any primary care physician may prescribe, and indeed this is what happens. 90% of prescribers are office based practice general practitioners. However, only 20% of such general practitioners have ever prescribed buprenorphine for the treatment of opioid dependence. Since registration of buprenorphine, in 1995, the number of patients in buprenorphine treatment has increased from less then 1000 to over 70,000 for an estimated 150,000 - 200,000 problem heroin using population. During the same time frame, reports of buprenorphine misuse have been published. This concerns in-treatment patients that inject their treatment, and out-of-treatment heroin or other pure mu agonists users, that occasionally will use buprenorphine. However, repeated measure studies of in-treatment patients have shown that buprenorphine treatment is related to a decrease of intravenous use, a decrease of polydrug use and an increase of social adjustment. Among polydrug users that favor the intravenous route, and are often out-of-treatment, and have had fatal outcomes, analytical toxicology have reported cases where buprenorphine was found. Because other drugs are also found, the role of buprenorphine in theses deaths remains unknown. At the same time, the overall mortality by overdose, that mainly concerns this polydrug intravenous out-of-treatment population has decreased by 78% from 1995 to 1998, and is steady at a 100 deaths per year since 1999.
