Afin de vérifier l'hypothèse d'un accroissement constant du nombre de patients alcoolodépendants hospitalisés sous contrainte, nous avons souhaité déterminer la place de l'alcoolodépendance dans l'utilisation de la loi du 27 juin 1990 dans le département du Puy-de-Dôme. Un travail de recherche rétrospectif reprenant 1 251 certificats médicaux d'hospitalisation à la demande d'un tiers (HDT) et 343 certificats médicaux d'hospitalisation d'office (HO) dans ce département entre 1992 et 1997 a permis de relever que 32 à 38 % des hospitalisations sous contrainte sont motivées par des problèmes d'alcool. De plus, à l'aide de ce travail, plusieurs caractéristiques spécifiques de l'hospitalisation sous contrainte des patients alcoolodépendants ont pu être dégagées : le motif d'hospitalisation en urgence de ces patients est un état d'agitation dans 45 % des cas et une altération de l'état général en rapport avec l'éthylisme dans 19 % des cas. La population des patients alcoolodépendants hospitalisés sous contrainte présente des singularités en ce qui concerne le genre (répartition homme/femme) et l'âge par rapport à la population des patients alcoolodépendants en hospitalisation libre. Les résultats de cette étude sont discutés et comparés aux données de la littérature. (Résumé d'auteur) ENGLISH : In order to verify the hypothesis of a growing increase in the number of involuntary admissions of alcohol-dependent patients, the authors evaluated the place of alcohol dependence in application of the French law of 27 June 1990 in the Puy-de-Dome department. A retrospective study based on 1,251 medical certificates for admissions requested by an interested person and 343 medical certificates for court-ordered admissions in this department between 1992 and 1997 showed that 32 % to 38 % of involuntary admissions were motivated by alcohol-related problems. This study also identified several specific characteristics of involuntary admission for alcohol-dependent patients: the indication for emergency admission of these patients was a state of agitation in 45 % of cases and alteration of the general state related to alcoholism in 19 of cases. The population of involuntary admission alcohol-dependent patients presents specific characteristics in terms of gender (male/female distribution) and age compared to the population of voluntary admission alcohol-dependent patients. The results of this study are discussed and compared in the light of the literature. (Author' s abstract) |
