L'intérêt de ce travail est d'effectuer une monographie d'un centre de cure ambulatoire en alcoologie (CCAA) pour étudier de plus près la démarche théorique et pratique que propose cette structure instituée. Le CCAA propose une alternative à l'hospitalisation lourde par un suivi pluriprofessionnel sur rendez-vous, au sein duquel le patient s'approprie un programme de soin. Le fait que l'institution soit souple dans le suivi, qu'elle ait ses murs propres, moins austères qu'un hôpital, et qu'elle maintienne la personne dans son environnement social, nous a poussés à nous interroger sur la manière dont elle parvenait à poser le problème alcool, à établir une relation de confiance avec le patient et à l'aider à se recomposer socialement. La confiance qu'elle instaure est une forme de réponse à la pathologie du lien et de la liberté que représente la dépendance au produit, réponse pouvant mener à une forme de dépendance envers la structure. Nous portons ainsi beaucoup d'intérêt à la relation du patient et de son entourage à la structure, au caractère négocié de la relation patient-praticien, à l'organisation entre praticiens pour aider le patient à s'approprier un programme de soin et, enfin, par la suite aux ajustements nécessaires du néo-abstinent à sa vie sociale. (Extrait du document.) ENGLISH : By the monograph of a CCAA, we suggest studying patient's medical and social trajectories accompanied for problems of alcohol. The CCAA articulates a set of competence susceptible to help the patient to recompose psychologically and socially to limit its suffering. Our approach suggests notably studying the work of the speakers of the centre in front of the variety of the met persons and the way they fit and exchange the information to direct the follow-up. By means of a direct observation made possible by the team, of conversations, of a participation in the meeting synthesis among practitioners and those opened to the patients and to their circle of acquaintances, we approach the work of the professionals and the realization of the undertaken link. This relation, it is an angle of approach, its bound to the pathology of the link and the freedom that characterizes the excessive alcoholization. Afterward, the objective is to have access to spontaneous testimonies of followed-up persons, which will give us concrete indications onto the spread strategies and the difficulties met in everyday life. This project will join a wider evaluation of the ambulatory device, including statistical study of the active row and theorical and practical comparison with the other centres. (Author's abstract.) |
