ENGLISH : Since the fall of the former Soviet Union, and the disruptive, political, economic and social changes which followed, there have been well documented outbreaks of HIV and other blood-borne infections among injecting drug users (IDUs) in the Russian Federation and elsewhere in Central and Eastern Europe. Research in The Russian Federation has shown that, as elsewhere in the world, needle and syringe exchange programs contributed to a decrease in HIV risk behaviour among IDUs. In this paper, Robert Power and his Russian colleagues describe the impact on clients of such harm reduction interventions in one region of the Russian Federation. They show that successful interventions such as NSEP and outreach can be implemented, individual behaviour change is evident among IDUs, an indigenous NGO sector has evolved, and the skills of workers have improved. Yet their optimism is tempered by ongoing systemic barriers to successful harm reduction which need to be addressed to prevent the spread of HIV and other blood-borne infections in the Russian Federation.
