Les malades adonnés aux opiacés ne changent guère de psychopathologie pendant six ans. Néanmoins les problèmes psychiatriques étaient nombreux dans la population observée et relevaient pour la plupart des troubles affectifs du DSM-III. Sur l'échelle de gravité de l'intoxication, mise au point par les auteurs, les facteurs psychiatriques se sont montrés les plus prédictifs. En cas de sévérité élevée à ces items, la réponse au traitement fournie par les centres de lutte contre la drogue et ceux de maintenance par la méthadone était faible et les rechutes fréquentes. Les auteurs insistent sur l'intérêt d'un diagnostic psychiatrique précis qui permette une thérapeutique appropriée susceptible d'apporter une amélioration. ENGLISH: An examination of substance abusers in contemporary practice reveals a high frequency of psychiatric disorders. In some cases, these probably represent preexisting conditions, but in others the disorder may be initiated or aggravated by the biologic and social consequences of substance abuse. The severity of a patient's psychiatric disorder is predictive of response to substance abuse treatment. A high proportion of affective disorders has been found among opioid-dependent patients; these are responsive to psychotherapy and to antidepressant treatment with doxepin. Careful psychiatric diagnosis in the management of substance abuse can discriminate those patients who are likely to require psychotherapy or psychoactive medication from those who may do well with standard drug counseling alone. |
