Résumé :

Points forts :

Highlights:
While the prevalence and overall volume of alcohol consumption decrease after the age of 65, drinking alcohol becomes more frequent - often daily - and the prevalence of alcohol use disorders among older adults remains stable.
Alcohol use poses a greater risk of harm and disease in older adults and is never recommended for the prevention of any medical condition.
Identifying and diagnosing alcohol use disorders is more challenging in this population, and requires adaptations of assessment tools and diagnostic criteria.
Treatment is feasible and beneficial at any age, even in the presence of physical or neurocognitive comorbidities, with prognoses remaining as good as - or even better than - those observed in the past. Withholding treatment based on age constitutes a form of ageism and may amount to neglect and mistreatment.
In addition to alcohol withdrawal (which requires tailored protocols), other approaches - including reducing alcohol consumption, harm reduction, and environmental modifications - should also be considered; these should be developed collaboratively with the patient (regardless of any impairments) and their personal and professional support networks.
