Objectif : Les opioïdes de synthèse, et en particulier le fentanyl, sont responsables d'une augmentation massive de la mortalité par intoxication en Amérique du Nord. En Europe et en France, cette problématique reste limitée mais fait l'objet d'une vigilance croissante. L'objectif de ce travail est de décrire une série de décès avec détection post-mortem de fentanyl dans un contexte médico-judiciaire afin de mettre en perspective ces observations dans le contexte national et européen actuel.
Méthodes : Une étude rétrospective descriptive a été menée sur 13 décès survenus entre 2020 et 2025 en incluant les cas présentant une détection sanguine de fentanyl hors contexte d'administration en service hospitalier. Les données démographiques, anamnestiques, médico-légales et toxicologiques ont été analysées. Les analyses toxicologiques reposaient sur un dépistage large par LC-MS/MS et LC-HRMS, et sur une quantification spécifique du fentanyl et du norfentanyl, le métabolite principal, dans le sang total par LC-MS/MS. Le rôle du fentanyl a été évalué par confrontation avec les concentrations mesurées, les substances associées et les éléments médico-légaux.
Résultats : Les sujets étaient tous de sexe masculin, âgés de 22 à 61 ans, et originaires d'Europe de l'Est. Tous les décès ont été observés dans le ressort du Tribunal judiciaire de Strasbourg. Une poly-consommation de substances psychoactives a été observée dans environ 70 % des cas, principalement avec des drogues illicites, des benzodiazépines et/ou des antipsychotiques. Les concentrations sanguines (sang périphérique) de fentanyl ont varié de 0,2 à 83ng/mL.
Conclusion : Cette série souligne la présence ponctuelle mais non négligeable du fentanyl dans des décès récents en région strasbourgeoise, principalement dans un contexte de poly-consommation. Elle met en évidence la nécessité d'une surveillance renforcée et d'une interprétation intégrative des données toxicologiques post-mortem. [Résumé d'auteur]

Objective: Synthetic opioids, particularly fentanyl, are responsible for a major increase in overdose-related mortality in North America. In Europe and France, this phenomenon remains limited but raises growing concern. The aim of this study was to describe a series of medico-legal deaths with post-mortem detection of fentanyl and to assess its role in the mechanism of death in the current national and European context.
Methods: A retrospective descriptive study was conducted on 13 medico-legal deaths investigated between 2020 and 2025 in which fentanyl was detected in post-mortem blood samples, whatever the context of its administration. Demographic, anamnestic, medico-legal and toxicological data were collected. Toxicological investigations included broad drug screening using LC-MS/MS and LC-HRMS, and targeted quantification of fentanyl and norfentanyl, its major metabolite, in whole blood by LC-MS/MS. The contribution of fentanyl to death was assessed by integrating blood concentrations, associated substances and medico-legal findings.
Results: All subjects were male, aged 22-61 years and originated from Eastern Europe. All cases were observed within the Judicial Court of Strasbourg. Polysubstance use was documented in approximately 70% of cases, mainly involving other illicit drugs, benzodiazepines and/or antipsychotics. Femoral blood fentanyl concentrations ranged from 0.2 to 83ng/mL.
Conclusion: This case series highlights the occasional but significant presence of fentanyl in recent medico-legal deaths in the Strasbourg area, predominantly in a context of polysubstance use. These findings support the need for continued surveillance and cautious, context-based interpretation of post-mortem fentanyl concentrations. [Author's abstract]
