Contexte : Les patients sous traitements de substitution aux opioïdes (TSO) suivis en CSAPA rencontrent de nombreux obstacles susceptibles de compromettre la continuité des soins. Le rôle potentiel des infirmiers en pratique avancée (IPA) dans cet accompagnement reste peu documenté.
Objectif : Décrire les besoins et difficultés rencontrés par des patients sous TSO suivis en CSAPA afin d'éclairer les enjeux organisationnels de leur accompagnement.
Méthode : Étude qualitative menée auprès de 20 patients (10 urbains, 10 ruraux) suivis dans deux CSAPA de la région Centre-Val de Loire. Des entretiens semi-directifs ont été réalisés puis analysés selon une approche thématique interprétative inspirée de l'analyse phénoménologique interprétative.
Résultats : Quatre axes principaux émergent : (1) obstacles d'accès et contraintes organisationnelles, particulièrement en milieu rural ; (2) ambivalence vis-à-vis du TSO, entre stabilisation et contrainte ; (3) importance de la relation thérapeutique, entre satisfaction et attentes d'écoute ; (4) impact du suivi et du traitement sur la vie quotidienne.
Discussion : Ces résultats mettent en évidence plusieurs enjeux organisationnels dans l'accompagnement des patients sous TSO, notamment en matière d'accessibilité du suivi, de continuité de l'accompagnement et de coordination des acteurs. Certaines compétences associées à l'exercice infirmier en pratique avancée pourraient constituer des pistes organisationnelles pour répondre à ces besoins.
Conclusion : Cette étude contribue à mieux comprendre les besoins exprimés par les patients sous TSO en CSAPA. L'impact du développement des pratiques avancées dans ce champ mériterait d'être évalué dans des travaux futurs. [Résumé d'auteur]

Background: Patients receiving opioid substitution treatment (OST) in addiction treatment centers (CSAPA) often face multiple barriers that may compromise continuity of care. The potential role of advanced practice nurses (APNs) in supporting these care pathways remains poorly documented.
Objective: To describe the needs and difficulties experienced by patients receiving OST in CSAPA in order to better understand the organizational challenges involved in their care.
Methods: A qualitative study was conducted with 20 patients (10 urban, 10 rural) followed in two CSAPA centers in the Centre-Val de Loire region (France). Semi-structured interviews were carried out and analyzed using a thematic interpretative approach inspired by interpretative phenomenological analysis.
Results: Four main themes emerged: (1) access barriers and organizational constraints, particularly in rural areas; (2) ambivalence toward OST, experienced both as stabilization and constraint; (3) the central role of the therapeutic relationship, between satisfaction and unmet expectations of listening; and (4) the impact of treatment and follow-up on daily life.
Discussion: These findings highlight several organizational challenges in supporting patients receiving OST, particularly regarding accessibility of care, continuity of follow-up, and coordination between professionals. Some competencies associated with advanced nursing practice may represent potential organizational approaches to address these needs.
