Le tabac est responsable de 13% des décès en France et est l'une des principales causes de maladies non transmissibles (MNT), telles que les cancers, les maladies respiratoires et cardiovasculaires. Pour renforcer sa politique de lutte antitabac, la France a mis en place un train de mesures entre 2016 et 2020 comprenant une augmentation progressive sur trois ans du prix des produits du tabac -correspondant à une augmentation de 41% du prix du paquet de cigarettes le plus vendu, le paquet neutre, une campagne annuelle de sevrage (#MoisSansTabac), et le remboursement des substituts nicotiniques. Ce rapport évalue l'impact sanitaire et économique du train de mesures de lutte antitabac, à l'aide du modèle de microsimulation de l'OCDE pour la planification stratégique de la santé publique pour les MNT (SPHeP-NCD), et il fournit une analyse de la charge du tabac en France et une évaluation économique de la campagne de marketing social pour le sevrage #MoisSansTabac.

Tobacco is responsible for 13% of all deaths in France and is a leading cause of noncommunicable diseases (NCDs). Although smoking rates have declined over time, a quarter of French adults (25.3%) smoked daily in 2021, which is above the OECD average of 16.5%. To strengthen its tobacco control policy, France has implemented a policy package between 2016 and 2020 aimed at controlling smoking initiation and boosting cessation. This package includes a three-year gradual increase in the price of tobacco products -corresponding to a 41% increase in the price of the most sold cigarette pack, plain packaging, a yearly cessation campaign (#MoisSansTabac), and the reimbursement of nicotine replacement products.
This report evaluates the health and economic impact of the 2016-2020 tobacco control policy package, using the OECD microsimulation model for Strategic Public Health Planning for NCDs (SPHePNCD). Implemented over the period 2023-2050, the policy package would avoid about 4 million cases of chronic diseases, save EUR 578 million per year in health expenditure, which is equivalent to 18% of the regional health budget of Bourgogne-Franche-Comté, or 4% of the amount France spent on preventive care in 2016, and increase labour productivity and participation in the labour market by the equivalent to 19 800 additional workers per year, compared to a scenario in which the intervention package would not be implemented. The cost of running these interventions - estimated at about EUR 148 million per year - is offset by the long-term savings in health care expenditure, with a return of EUR 4 for each euro invested in the policy package. The report also provides an analysis of the burden of tobacco in France and an economic assessment of the social marketing campaign for cessation #MoisSansTabac.
