Buprenorphine has some advantages over methadone, such as missing evidence for QTc prolongation, possible alternate day dosing, limited respiratory depression, and a lower risk of overdosing. A transition from methadone to buprenorphine can become necessary for many reasons. There is presently no standard protocol for transition from high-dose methadone to buprenorphine.

The aim of this study was, therefore, to examine methods that could be used to give patients in higher dose methadone treatment the possibility to switch to buprenorphine without a prior reduction of the dose, decrease withdrawal discomfort, and keep the transfer time period as short as possible.
