The handbook is divided into seven sections. The first section examines the spread of AIDS through the United States, with special emphasis on the impact of AIDS outside the disease epicenters and in Puerto Rico. The second section explores the role of needle use patterns and behaviors in the spread of AIDS. The emergence of crack and its increasing significance for the spread of HIV infection are examined as well. The third section examines gender issues as related to HIV spread and injection drug use. The characteristics of female sexual partners are explored in depth, and the special issue of the exchange of sex for drugs is also detailed. In the fourth section ethnic and age differences in the functioning and behavior of injection drug users are discussed. The role of homelessness and the influence of mobility are each reviewed. A substantial minority of injection drug users (up to 20% in some cities) have to cope with issues of homelessness in addition to addiction, and the issue of travel to different cities by injection drug users is of obvious concern to the spread of disease. The nature and frequency of criminal activity in the out-of-treatment population are examined as well. The fifth section describes different issues in the use of drug abuse treatment by the injection drug-using population. The study compares the characteristics of drug users reached by outreach programs and injection drug users electing to enter treatment on their own. In the sixth section, specific outreach and intervention strategies are explored. The use of indigenous workers to reach injection drug users is described, as is the effort to reach sexual partners, using workers familiar with the communities and agencies from which sexual partners are recruited. In addition, the role that treatment programs can play in outreach to injection drug users is examined in comparison to the role played by community-based organizations. Finally, the too often overlooked contribution of 12 step programs is explored. The final section examines the impact of behavior change strategies on drug use, needle using, and sexual behaviors. The section explores the overall efficacy of interventions with injection drug users, as well as the contribution of specific initiatives and components of programs. (Extract of the publication)
