A partir d'une revue de la littérature (68 réf.), l'auteur effectue une synthèse des connaissances accumulées après 30 ans d'expérience clinique et de recherche dans le domaine des maintenances à la méthadone. Sont analysés l'impact dans l'efficacité de différents facteurs liés au traitement, ainsi que les corrélations entre facteurs-patient et résultats. Parmi ces derniers, il est particulièrement question de psychopathologie, sujet à partir duquel est développée la question des approches thérapeutiques parallèles à l'administration de méthadone, leurs indications et leur efficacité. ENGLISH :
From a review of the litterature, the author proposes a synthesis about clinical experiences and researches in methadone maintenances during this 30 last years. Treatment factors and patient factors are analysed in relationship with the efficacy of the treatments. One of the most relevant patients-factors is linked with the psychopathology ; psychosocial services added to the pharmacotherapy are analysed in term of indication and efficacy. The main outcome are : retention, reduction of heroin consumption, harm reduction for injections diseases, decrease of criminality and mortality. The effectiveness is correlated with : long-term maintenance, stable teams ; friendly and supportive relationship with patients ; individualised doses (more > 60 mg) with psychosocial and/or psychotherapy in cases of comorbidity.
