Modified therapeutic community treatment for homeless dually diagnosed men. Who completes treatment?

Dans le cadre d'une communauté thérapeutique adaptée, à Bellevue Hospital (New-York), les caractéristiques de 189 hommes sans domicile souffrant de troubles d'abus et de troubles psychiatriques, ont été examinées dans le but d'identifier celles prédisant l'achévement des 6 mois de traitement. 34% des patients ont mené à terme le programme, et principalement deux variables significatives paraissent prédire leur réussite dans le programme : un faible nombre d'hospitalisations psychiatriques antérieures, et un plus grand nombre d'activités professionnelles. Par rapport aux communautés thérapeutiques traditionnelles, la délinquance apparaît moins importante comme facteur prédisant la réussite, et les taux de réussite semblent similaires.

The authors studied a modified therapeutic community designed for the treatment of patients with combined substance abuse and psychiatric disorders. This model has been applied on a limited basis in clinical practice, and little is known about the characteristics of patients who are likely to complete the prescribed stay in such a program. In this report, the authors present characteristics of 189 homeless dually diagnosed men who entered a shelter-based, modified therapeutic community with a prescribed 6-month stay. 34% of admissions completed the prescribed stay. These patients were more likely to have fewer inpatient psychiatric admissions and more job experience than those who did not complete their stay. Findings are discussed in terms of their similarities and differences to findings from traditional therapeutic communities for the singly diagnosed. (Authors'abtract)
