2008 could be a landmark year for progress on a vexing global public-health problem, the use of illicit drugs. Beginning in March, member states of the UN have been asked to engage in a year of reflection to gauge progress in the decade since the 1998 UN General Assembly Special Session on the problem of drugs worldwide. The 1998 session was convened under the catch-phrase A drug-free world - we can do it. The declaration from the meeting emphasised the goal of complete eradication of opium poppy, coca, and cannabis production and illicit drug use, with a focus on policing and criminal law. However, the declaration was largely silent on measures to address HIV infection and other health problems for people who are unable or unwilling to stop using drugs. In 2008, the UN must do better. Epidemics of HIV and hepatitis C driven by injected-drug use should weigh heavily in the considerations of member states on whether they really can - or should try to - achieve a drug-free world.
