L'auteur compare l'obligation de traitement dans les différentes législations en vigueur dans les pays nordiques, Danemark, Finlande, Norvège et Suède. Il cite le système paternaliste (c'est pour son bien) et à l'opposé, le traitement qui respecte l'intérêt du toxicomane. Il explore aussi la théorie du moindre mal : l'emprisonnement quand on estime que c'est la seule manière d'éviter à la personne une maladie grave ou la mort. Trois types proches de systèmes sont également étudiés : le placement d'office en service psychiatrique, l'emprisonnement pour état d'ivresse et le traitement des toxicomanes en probation. En conclusion il apparaît que compte tenu des différences importantes entre les pays, on ne puisse pas parler de "modèle nordique".

One of author's theoretical premises is the concept of paternalistic involuntary treatment, i.e. treatment for the individual's own sake. At the opposite pole of paternalism is utilitarian involuntary treatment, for instance, where the chief purpose is to protect the relatives of the substance abuser. Lehto also explores the concept of "the least evil" which, in the case of involuntary treatment, implies that the individual is taken into custody because that is seen as the only one way of sparing him from serious illness or death. The analysis is not restricted to involuntary treatment within the substance abuse system, but it also takes in three closely related systems : psychiatric committal, arrests for drunkenness and treatment for substance abusers within the probation and after-care system. There are huge differencies between the four Nordic countries. Thus, in the field of involuntary treatment of alcohol and drug problems, there is no uniform "Nordic model"
