La clinique montre un déficit de paroles dans l'histoire de sujets toxicomanes. La rencontre avec des toxiques sera le moyen de s'évader d'un "silence familial", de joindre ce qui leur est inaccessible par les mots, de trouver des satisfactions par l'auto-érotisme. L'auteur se préoccupe de repérer ce qui fait bascule de l'accueil aux soins en consultation. A partir d'une étude de cas, il tente une modélisation des premiers entretiens et de la mise au travail psychique en utilisant la formalisation des discours posée par LACAN. L'article se termine sur le schéma : "les quatre discours" (discours du maître, de l'analyste, de l'hystérique, et de l'Université) qui résume son raisonnement.

Clinic shows a speech deficit in the drug addicts' history. Taking drugs would be the only mean to escape onseself from a " family silence", to join what is inaccessible to oneself by the words, and to find some satisfactions by auto-eroticism. The author tries to spot what topples from the reception to the care at the consultation. From a case study, he tries to make a modelisation of the first discussions and the setting up of the psychological work by using the formalisation of Lacan speeches. The article ends up with the outline : " The four speeches" (speech of the master, of the analyst, of the hysteric and of the University) which summarize his reasoning.
