Les traitements de substitution en matière de toxicomanies se voient prendre aujourd'hui en France un essor rapide, impulsé essentiellement par une politique de réduction des risques. A partir d'une réflexion sur les spécificités de ce type de traitements, sur certains aspects pathogéniques des conduites toxicomaniaques, et sur les difficultés éprouvées par le toxicomane à se libérer de sa dépendance, il semble opportun de s'interroger sur le statut de la substitution dans la relation soignant-soigné et sur les possibilités d'inscription du traitement de substitution comme médiation à une approche psychothérapique de l'addiction. Nowadays, the treatments of substitution are in full expansion, mainly impelled by a policy of reduction of the risks. Starting from a reflection about the specificities of this type of treatments, about some pathogenic sides of drug addict behaviour and about the difficulties that the drug addict person have to releae themselves from their dependance, then it would be appropriate to wonder about the status of substitution in the realtionship between the one who treats and the one who is treated, and about the possibilities of inscription of the treatment of substitution as a mediation in a psychotherapeutic approach of addiction. Les traitements de substitution en matière de toxicomanies se voient prendre aujourd'hui en France un essor rapide, impulsé essentiellement par une politique de réduction des risques.
