Recruitment into treatment of homeless, mentally ill, chemical abusing men

Supposant que le traitement de la toxicomanie commence en fait bien avant que les toxicomanes ne s'engagent dans un programme de traitement, cet article conceptualise le processus d'initialisation du traitement et étudie l'attrition des clients pendant cette phase d'initialisation. L'article identifie deux phases avant l'engagement dans le traitement, l'exploration du traitement et l'initialisation du traitement, et rapporte les résultats d'une étude sur 1924 sans abri, de sexe masculin, présentant des troubles mentaux et toxicomanes qui cherchaient un traitement basé sur une communauté dans New York City entre 1991 et 1996. Seulement 326 de ces hommes ont en fait débuté le traitement. Les autres ont été perdus soit avant, soit pendant la phase d'initialisation. L'article insiste sur les 823 hommes qui ont atteint la phase d'initialisation et tente de relier leurs caractéristiques sociodémographiques, psychologiques et toxicomaniaques avec les différents types d'attribution pendant l'initialisation. Les résultats montrent que certaines caractéristiques des clients sont prédictives du rejet, d'autres de l'acceptation dans les programmes de traitement.

Under the assumption that the treatment of substance abuse begins well before subtance abusers actually enroll in treatment, this paper conceptualizes the process of recruitment into treatment and investigates attrition of treatment, seeking clients during the treatment recruitment stage. The paper identifies two stages prior to treatment enrollment, treatment exploration and treatment recruitment, and presents the result of a study of 1,924 homeless, mentally ill, chemical abusing men who looked for community-based treatment in New York City between 1991 and 1996. Only 326 of these men actually entered treatment. The rest were lost either prior to or during the recruitment stage. The paper focuses on the 823 men who reached the treatment recruitment stage, and attempts to correlate their sociodemographic, psychological, and substance abuse characteristics with the different types of attrition during treatment recrutment. The results show that certain client characteristic predict rejection and certain other characteristics predict acceptance by the treatment program. (Authors'abstract)
