Dans le but d'aider les obstétriciens, et autres praticiens, dans leur approche de patientes enceintes consommant des substances, cet article présente d'abord l'étendue du problème au niveau épidémiologique, social et économique, puis il aborde les principales questions éthiques et juridiques, en terme politique, et expose des cas connus liés aux droits du foetus et à la consommation maternelle de substance. Enfin, une intervention clinique axée sur l'éducation et l'amélioration de la reconnaissance et de la gestion de cette population est proposée.
ENGLISH :
Substance use in pregnancy has garnered increasing attention over the last decade as a particularly concerning facet of the larger national drug problem. This article is intended to assist obstetricians and others in their approach to the substance using pregnant patient. The authors first review the scope of this problem in social and financial terms and then review the important ethical and legal issues involved in current policymaking. Lastly, they suggest a clinical intervention focusing on education and improvement in identification and management of this subset of patients. (From authors' abstract)
