Génétique et alcoolisme : intérêt pratique en clinique ?

La recherche génétique sur l'alcoolisme peut concerner directement le clinicien dans la prise en charge de son patient. Parmi les avancées majeures de la recherche dans cette pathologie complexe, nous décrivons dans cette revues six données qui peuvent enrichir la prise en charge par le clinicien de son patient alcoolique. Ces données concernent : (1) le lien entre une concentration familiale d'alcoolisme et le risque individuel pour cette pathologie ; (2) l'impact des antécédents familiaux sur le type d'alcoolisme dont souffre le patient ; (3) le poids de l'histoire familiale sur la sévérité et le pronostic de l'alcoolisme ; (4) la notion de vulnérabilité génétique croisée pour les différentes dépendances (drogue et tabac notamment) ; (5) le rôle interactif de l'environnement familial et de la vulné- rabilité génétique dans la compréhension des déterminants de la maladie et des rechutes ; (6) enfin, le concept de «protection génétique» pour la maladie. Ces différentes notions viennent souligner que la recherche des antécédents familiaux d'alcoolisme doit faire partie de l'évaluation globale et de la prise en charge d'un patient ayant un problème avec l'alcool. ENGLISH :
Research on the genetics of alcoholism may concern the clinician in his common practice. Apart from keeping in touch with recent advances on one of the most important field of research on alcoholism (namely genetics), we describe in this review six domains in which looking for familial history of alcoholism may help to take in charge alcohol-dependent patients. Firstly considering the link between familial bakground and personal risk for alcoholism. Secondly looking for the impact of familial history on the type of alcoholism. Thirdly describing the involvment of familial history on severity and prognosis of this disorder. Fourthly taking into account the existence of a cross-vulnerability for the different addictions (specifically, for tobacco and drug). Fifthly, regarding the interactions between familial environment and genetic vulnerability in the comprehension of alcoholism determinants. Lastly describing the concept of genetic protection against alcoholism. These domains highlight the importance of looking for familial history of alcoholism in the global care given to patients with alcohol abuse and/or dependence. (Author's abstract.)
