Plaidoyer pour la prescription d'héroïne

Prioritaire dans ces années 90, la vision de santé publique met en avant une stratégie collective de soin : la prévention des risques par la généralisation des produits de substitution. Mais le toxicomane n'aurait-il que deux choix : l'abstinence ou la substitution ? Ce qui est en jeu dans la substitution est l'abstinence des drogues illicites. Nous n'adhérons pas à la vision péjorative de l'irresponsabilité du toxicomane. Si nous pouvons envisager la rechute comme partie intégrante de son chemin thérapeutique, comment ne pas rajouter à nos différentes stratégies la distribution médicalisée d'héroïne ? Après avoir exposé les expériences anglaises et suisses de distribution d'héroïne, l'auteur développe la proposition d'une prescription cadrée en France, comme une possibilité thérapeutique parmi d'autres, ayant l'avantage de redonner une liberté de choix au toxicomane, de défendre une certaine approche individuelle des patients. The public health approach, which was considered a priority in the 1990s, placed the emphasis on a collective treatment strategy : the prevention of risks by the widespread use of substitution products. But are there really only two alternatives for the drug user : abstinence or Substitution ? What really counts in substitution is abstinence from illegal drugs. We do not adhere to the derogatory view that drug users are irresponsible. If we can accept relapses as a normal part of the therapeutic process, how can we omit the medical distribution of heroine from our different strategies ? Having described the English and Swiss experiences in distributing heroine, the author outlines a proposal for controlled Prescription in France as a therapeutic possibility among others, which would have the advantage of giving freedom of choise back to the drug user and defending a certain individual approach to the patient.
