Dihydrocodeine: a useful tool in the detoxification of methadone-maintained patients

Les avantages présentés par la dihydrocodéine dans le sevrage de 20 patients sous traitement de maintenance à la méthadone ont été étudiés. 65% des patients avaient terminé avec succès leur traitement de désintoxication et étaient abstinents à la fois de la méthadone et de toute autre drogue opiacée au bout de deux semaines de programme. En complément, trois patients ont entamé un traitement à la naltrexone et un autre était abstinent après un suivi dune semaine supplémentaire. Un patient a rechuté mais est resté en contact avec lunité de désintoxication. Les six patients restant ont abandonné le programme au bout de trois et 11 jours. En conclusion, les avantages de la dihydrocodéine dans la désintoxication des patients sous maintenance à la méthadone semblent partiellement démontrés.

We investigated the merit of dihydrocodeine tartrate for withdrawal in detoxifying 20 methadone-maintained patients presenting for treatment at the Leeds Addiction Unit (LAU). Thirteen patients (65%) successfully completed methadone detoxification and were abstinent from both methadone and opiate-type drugs at the end of the 2-week program. On completion, three patients began treatment with Naltrexone and another was abstinent at a follow-up appointment, I week later. A further patient relapsed back to heroin use but remained in contact with the LAU. The remaining six patients dropped out of the 2-week detoxification program between days 3 and 11 of the dihydrocodeine cross-over period. We believe dihydrocodeine may have advantages in detoxifying methadone-maintained patients. (Editor's abstract.)
