Improving the quality of substance dependency treatment with pharmacotherapy

Revue des traitements pharmacologiques standards et nouveaux pour traiter la dépendance à l'alcool, aux opioïdes, à la cocaïne et à la nicotine. Des directives pour traiter un patient présentant plusieurs dépendances sont indiquées ainsi que des recommandations sur les moments où une pharmacothérapie doit être administrée. Par ailleurs, les auteurs indiquent des thérapies particulièrement utiles pour traiter les patients présentant à la fois une dépendance aux substances psychoactives et des troubles psychiatriques. Afin de maximaliser les bénéfices thérapeutiques contre la dépendance aux substances psychoactives, les auteurs préconisent que les patients soient individuellement évalués. Des recommandations pour la recherche en laboratoire et/ou en clinique sont données.
ENGLISH :
This article provides an overview of current pharmacological treat- ments for alcohol, opioid, cocaine, and nicotine use disorders. Guidelines for a "patient-treatment" matching framework to physicians working with various "substance-abusing" patients are presented, as well as recommendations regarding when to initiate and discontinue pharmacotherapy. Standard and newer pharmacological treatments for substance dependence are reviewed, as well as therapies that may be especially useful when treating the patient with comorbid substance dependency and psychiatric disorders. To maximize the therapeutic benefits of substance dependency treat- ment, patients should be individually assessed and provided adjunctive medications as clinically indicated.
