"Are you for harm reduction or for abstinence?" asked the young medical student. Surprised at the incongruity of the question, I asked her to explain how she came to view those two concepts as opposing one another. Her response left me feeling rather sad and worried about the message of helplessness which appears to have influenced her thinking. Here was an enthusiastic clinician in training, avidly learning about therapeutic methods to bring sick people back to health, and what did she understand the treatment of addictions to be? Nothing more ambitious than retention in therapy and prevention of avoidable complications. In fact, she had heard in Public Health seminars that the "traditional" therapy goal of drug-free living was often unrealistic, and that it usually backfired, making things worse. Thus, hers was not an uninformed assumption, but a reflection of the teaching she was receiving. [Extract]
