Résumé :
Plasma methadone concentration was determined in 71 patients in methadone program by enzymoimmunoassay tecnique (EIA) for 15 month to assess their utility to predict adequate dosage and relationship with clinical state during this time, related to opioid use, opioid withdrawal symptoms, associated illness (HIV and Hepatitis) and pharmacological interaction of medications.There was a correlation between opioid withdrawal symptoms and plasma levels decrease p<0.01 and relapsed in heroin use (33.5%). On the other hand, patients with benzodiazepines took higher doses (124 mg vs. 86 mg). Neither plasma concentrations nor doses were significantly different in HIV + patients, however, HCV patients needed higher doses to get a similar plasma level (99 mg vs 73 mg). Neither plasma concentrations nor doses, were significantly different between patients with or without medication when the study started. After that, patients had an increase of doses (145 vs. 81.6+31.2) p< 0.01 with significant differences since 3rd blood sampling, with decreased plasma levels (145+58 vs. 81.6+31.2) p<0.01 with the induction effect on the metabolism of methadone of prescribed medication. So "Adequate Plasma levels", more than adequate doses can be used like a clinical instrument to assess methadone treatment efficiency and pharmacological interaction in stable patients.
