The use of tobacco, alcohol, and other drugs is probably linked to nearly a fourth of annual deaths that occur in the Ameri- cas. In the Southern Cone of Latin America, close to 40% of teenagers already smoke by age 15. The great majority of them breath at home air polluted with second hand smoke. In Latin America each person consumes 6 kilograms of pure alcohol each year on average, the highest figure in the less developed world. Despite the fact that data on use of illegal drugs is not widely available, it is known that marihuana is the drug of choice of a high proportion of the population. It is estimated that 45 million people consumes marihuana in the Americas. It is true that the disease burden generated by illegal drugs is not as high as that generated by legal drugs. However, their social consequences are higher. The principal challenges for the control of smoking and of the use of alcohol in the Americas are two: diminish the availability and affordability of the products, and to avoid the exposure of the population to the marketing practices of the tobacco and alcohol industries. Furthermore, in the case of tobacco, it is necessary to protect non-smokers from the smoke of others. In the case of the illegal substances, the results until now of an approach focused on supply control have been limited. The challenge is to emphasize the control of the demand through interventions of prevention, detoxification, treatment, and rehabilitation, as well as harm reduction. (Review's abstract.)
