Pour évaluer la prise en charge des patients fumeurs par les médecins généralistes, on interroge les médecins sur leur tabagisme personnel, leur stratégie de dépistage et l'aide qu'ils proposent aux patients. 85% des médecins non-fumeurs interrogent leurs patients sur leurs habitudes tabagiques contre 64% des médecins fumeurs. Les femmes dépistent mieux que les hommes. Les tests de Fagerström, HAD et de motivation sont relativement peu employés. 3% des praticiens font une exploration fonctionnelle respiratoire ce qui reste très insuffisant, un médecin sur deux ne connait pas l'adresse des consultations de tabacologie de sa région.
ENGLISH :
OBJECTIVE: to estimate the assessment of the patient smokers by the GPs. Physicians were also interrogated on their personnal smoking, the strategy used to screen for tobacco use and its respiratory consequences, the treatments they used for smoking cessation, and on the difficulties encountered in their initial and further medical training in this domain. RESULTS: the effective response rate was high (75%). According to the doctor's answer, 85% of non-smoking physicians interrogated systematically their patients about their smoking behaviour, and only 64% among the smoking physicians (p<0,034). The ex-smoking physicians declared fewer problems during a consultation for smoking cessation than their non-smoking and smoking colleagues (p<0,048). However, few general practitioners used the validated tests: Fagerström (26%), HAD (2%), Motivation (8%). Only 3% of the practitioners declared to systematically prescribe a respiratory functional exploration to evaluate the respiratory status of their patients. Fifteen percents of the doctors considered that the lack of recognition and sufficient retribution for smoking cessation has a negative impact on the correct assessment of the patients. Only half of the practitioners knew the adress of out patient tabacology unit of their city. (Author' s abstract)
