Le passage à l'urgence est un moment privilégié d'intervention auprès du toxicomane et de son entourage. A cette occasion, en effet, le psychiatre et son équipe peuvent proposer au patient et aux aidants naturels de nouvelles règles interpersonnelles pour transformer des relations explosives en des rapports de coopération. L'auteure, à l'aide de vignettes cliniques, met en évidence les éléments de l'ensemble protectionniste qui, une fois bien compris, permet à l'équipe d'éviter de rejeter le toxicomane. Elle décrit les étapes de son intervention, qui se veut multisystémique, et souligne l'importance des notions de leadership, de processus et de réseautage lorsque rien ne va plus. Eviter les mesures aversives envers le toxicomane et favoriser plutôt un processus de réorganisation avec son réseau personnel à partir des capacités et des limites de chacun, voilà l'idée essentielle de l'intervention proposée. ENGLISH:
The Emergency visit is a propitious moment for initiating an intervention with those persons (including their support network), who have drug and/or alcohol problems. It is a timely moment for the psychiatric team to propose a new start in their communication process and to facilitate the transformation of explosive relationships into co-operative ones. The author exposes the elements related to her protectionist model, through the use of vignettes, which once understood, assist the team in reducing the incidence of rejection in this drug and alcohol population. She describes the steps of a multisystemic intervention and stresses the importance of the need for leadership qualities and of creating a support system when nothing else seems manageable. The idea is to avoid plunging the person, with drug and/or alcohol abuse further into a no-win situation. The essence of the intervention is to favour a relationship change with the patient’s personal network based on the limitations and capabilities of everyone involved in the process.
