Injecting drug use (IDU) by sharing non sterile needles and syringes has been clearly related to the spread of the HIV infection and the development of AIDS since the onset of the epidemic, specially in Southern European countries and specifically in Spain and a few years after in Portugal. Nowadays some Eastern European countries, like the Ukraine, are being increasingly affected, and IDU constitutes a major way of transmission in almost all parts of the world. The infection's evolution in Spain shows a different development in the various autonomous communities. Whereas some of them, like Madrid or the Basque Country, have high AIDS rates in injecting drug users, others, like the Canary Islands, show rather lower numbers. In this scene harm reduction strategies, like the distribution of sterile needles and syringes, needle exchange and the availability of opiate derivatives treatment, have been largely recognized as fairly efficient worldwide.
