Depuis 2008, le baclofène est utilisé hors autorisation de mise sur le marché, et parfois à hautes doses, dans le trouble d'usage d'alcool (TUAL). L'encadrement réglementaire par la recommandation temporaire d'utilisation (RTU) de 2014 a donné à ce traitement une légitimité attendue par les prescripteurs et les associations de patients. Dans l'attente des résultats des essais cliniques d'efficacité, annoncés pour ce mois de septembre, de nombreuses questions de sécurité restent à préciser. L'étude BACLOPHONE, récemment commencée en région Hauts-de-France, est une étude de pharmacovigilance téléphonique qui doit être réalisée auprès de 800 patients, au moment de l'instauration de leur traitement par baclofène pour TUAL. Ils sont suivis pendant 1 an ou jusqu'à l'arrêt de ce traitement. On étudie en détail tous les événements indésirables afin d'en déterminer l'imputabilité exacte. On note donc, au jour le jour, pour chaque patient, les consommations d'alcool et d'autres substances associées et les doses de tous les traitements. À noter : les patients inclus ne correspondent pas forcément aux exigences de la RTU. Tous les prescripteurs de baclofène de la région peuvent proposer à leurs patients d'être inclus dans l'étude. Au-delà de l'intérêt scientifique propre de l'étude, une rémunération financière est prévue pour les médecins qui incluent leurs patients. ENGLISH: Since 2008, baclofen has been used off-label, occasionally using high doses, for treating alcohol use disorder (AUD). The regulatory supervision of the 2014 temporary recommendation for use (TRU) measure has given baclofen a legitimacy that was awaited by both the prescribers and the patient associations. While the results of the efficacy clinical trials have been announced for next September, numerous safety issues are still to be addressed with regard to this medication. The BACLOPHONE study has recently started in the Hauts-de-France region. BACLOPHONE is a phone-based pharmacovigilance conducted among almost 800 patients in whom a baclofen prescription has been started for AUD. Patients will be followed-up during 1 year, or until baclofen has been stopped. All occurring adverse events are thoroughly studied, with the aim to assess the exact causality of baclofen. Day by day drinking patterns, baclofen and other drug doses, and other substance uses, are systematically noted. The fulfilment to the TRU criteria is not required for including patients in the study. Every baclofen prescribers of the Hauts-de-France region can propose to their patients to be included in BACLOPHONE. Beyond the mere scientific prospects of the study, a financial remuneration is provided to the prescribers who will include their patients in the study. |
