La réduction du tabagisme dans les populations socialement défavorisées est une composante importante de la réduction des inégalités sociales de santé. Pour cela, outre la prévention de l'initiation tabagique au sein de ces populations, il importe de concevoir des interventions d'aide à l'arrêt du tabac efficaces et adaptées à cet objectif. Le projet StopAdvisor, mené par une équipe britannique, est un dispositif d'aide à l'arrêt du tabac par Internet. Cet article propose la synthèse d'un ensemble de travaux consacrés à la conception et l'évaluation de StopAdvisor. La conception du site a reposé sur la théorie de la motivation PRIME (Plans, Responses, Impulses, Motives, Evaluations) de Robert West ; il a été testé auprès de fumeurs britanniques socialement défavorisés (ayant un faible niveau d'études, au chômage ou ayant le statut d'ouvriers). Un essai contrôlé randomisé a été réalisé auprès de 4 613 fumeurs, pour moitié socialement défavorisés. Le critère d'analyse principal était une abstinence continue de 6 mois vérifiée biologiquement. L'étude a permis de montrer que le dispositif interactif StopAdvisor était plus efficace qu'un simple site d'information statique auprès des fumeurs socialement défavorisés, alors qu'il ne l'était pas pour les fumeurs favorisés. Pour les fumeurs ne souhaitant pas s'engager dans un suivi psychologique en face-à-face, un dispositif d'aide à l'arrêt par Internet pourrait donc constituer une alternative efficace, notamment pour les fumeurs ayant un faible niveau socioéconomique. ENGLISH: Reducing smoking among smokers from low socioeconomic status is an important component for reducing social inequalities in health. To achieve this, besides the prevention of smoking initiation in these populations, it is important to design effective and appropriate interventions to help them to stop smoking. The StopAdvisor project, led by a British team, is a smoking cessation website. This article is a synthesis of a series of original articles devoted to the design and the evaluation of StopAdvisor. The website was designed according to Robert West's PRIME (Plans, Responses, Impulses, Motives, Evaluations) motivation theory, and was tested by unemployed smokers or smokers with a low education level or a low economic status. A randomized controlled trial was conducted among 4,613 smokers, half of whom were of low socioeconomic status. The primary analysis endpoint was continuous abstinence for 6 months, and was biochemically verified. The study demonstrated that StopAdvisor was more effective than a simple information static webpage for low socioeconomic status smokers, while it was not among smokers with a higher socioeconomic level. For smokers who do not wish to engage in face-to-face psychological counseling, a web-based mechanism may therefore be an effective alternative, in particular for smokers with a lower socioeconomic status. |
