Introduction. Les conséquences pour le développement foetal d'une exposition à l'alcool in utero varient en fonction de l'intensité de l'exposition. Il n'a pas été mis en évidence une dose -seuil en deçà de laquelle une consommation d'alcool par une femme enceinte serait inoffensive. Malgré les recommandations d'abstinence totale, le pourcentage de femmes déclarant a posteriori avoir eu un contact avec l'alcool au cours de la grossesse reste relativement élevé. Nous avons souhaité évaluer la performance d'un autoquestionnaire pour le dépistage de ces consommations. Ce questionnaire devait aussi évaluer l'information reçue par les femmes à propos des dangers à consommer de l'alcool durant la grossesse. Méthode. Le questionnaire a été distribué à 500 patientes aux consultations obstétricales de deux établissements hospitaliers. Résultats. 63,4 % des questionnaires nous ont été retournés ; 6,3 % des répondantes déclaraient une consommation d'alcool durant leur grossesse ; 77 % s'estimaient suffisamment informées sur la question, et plus de la moitié citaient les professionnels de santé comme source d'information. La satisfaction quant à l'information reçue était significativement liée à des comportements de diminution ou d'arrêt des consommations d'alcool. Discussion. La représentativité de l'échantillon est bonne, avec des caractéristiques proches de la population générale. La sensibilité de notre questionnaire est supérieure à celle de l'entretien obstétrical standard mais reste en dessous des évaluations réalisées a posteriori (enquête périnatale). La recommandation d'abstinence et les raisons de cette recommandation sont relativement méconnues du grand public. L'accent devrait donc être mis sur la prévention et l'information systématique. ENGLISH: Introduction. Foetal consequences of an alcoholic exposure in utero may vary according to the intensity of that exposure. A dose-threshold below which alcohol consumption by a pregnant woman would be harmless has not been highlighted. In spite of the recommendations of total abstinence, women's percentage declaring a posteriori to have had a contact with alcohol during pregnancy remains relatively high. We intended to estimate the performance of an autoquestionnaire for the screening of these consumptions. This questionnaire also had to assess the information received by the women about the dangers to consume alcohol during pregnancy. Method. The questionnaire was distributed to 500 patients in the obstetric consultations of two hospitals Results. 63,4% of questionnaires were returned to us; 6,3% of the patients declared an alcohol consumption during pregnancy; 77% considered themselves as sufficiently informed about the question and more than hall quoted the healthcare professionals as the information source. The satisfaction as for the received information was significantly connected to behaviour of reducing or giving up the alcohol consumption. Discussion. The representativeness of the sample is good with characteristics close to the general population. The sensibility of our questionnaire is superior to that of the standard obstetric interview but stays below the a posteriori evaluations (perinatal investigation). The recommendation of abstinence and the reasons of this recommendation remain relatively underestimated by the general public. The accent should consequently be put on prevention and systematic information. |
