À travers une réflexion sur les « contraintes par corps », sur le statut du toxique en tant que pharmakon (à la fois poison et remède), ainsi que sur les « identifications addictives inconscientes », l'auteur interroge la manière dont les sujets addictés tentent de se façonner « un corps de substitution ». Il s'agit plus précisément de montrer comment le processus d'addiction vise le façonnement d'un nouveau corps : « se faire un corps étranger » en incorporant un « corps étranger toxique » représente une tentative d'automédication, afin d'éloigner l'emprise d'un autre primordial. C'est à partir de tous ces éléments cliniques et théoriques développés dans cet article que l'on peut questionner d'une nouvelle manière le statut des « traitements de substitution » prescrits par des médecins aux patients addictés, ainsi que leur articulation avec les psychothérapies proposées par des psychologues cliniciens. ENGLISH: Through a reflection on "contrainte par corps" (civil imprisonment, litterally: bodily constraint), and on the "unconscious addictive identifications", the author questions the manner in which addicted subjects try to build up a "substitution body". Our intention is to demonstrate how the addictive process aims at the construction of a new body: by making a "foreign body", incorporating a toxic foreign body is an intent to self-medicate, in order to remove the hold of a primordial other. All these clinical and theoretical elements, developed in this article, raise the question of the status of the "substitutive treatments" that medical doctors prescribe to addicted patients, as the articulation of these with therapies proposed by clinical psychologists.
