FRANÇAIS : La combinaison d'un modèle comportemental de renforcement de comportements positifs et des méthodes d'entraide (les 12 phases de AA et NA) ont permis de gérer un programme intensif de traitement pour patients à double diagnostic (psychiatrique et toxicomaniaque) dans le cadre d'un service d'urgence psychiatrique d'un hôpital municipal. Le programme encourage le patient à participer activement à un processus de rétablissement qui s'attaque simultanément à sa maladie mentale et à sa toxicomanie. Les difficultés cliniques et organisationnelles rencontrées lors de la mise en place du programme sont décrites. Elles concernent entre autres le manque de motivation de la population de patients concernée et l'absence de formation en toxicomanie chez l'équipe psychiatrique gérant le programme. Le programme a fonctionné pendant 5 ans et plus de 1000 patients ont été traités. La mise en place du programme comportemental a fait diminuer considérablement les problèmes de violence dans le service et a amené une augmentation du taux de participation des patients à des activités de groupe ENGLISH : The combination of a token economy and a self-help model made it feasible to organize and maintain an intensive dual-diagnosis treatment program in a public municipal hospital acute psychiatric ward. The program described here, which integrates the token economy and 12-step approaches, fosters voluntary and active patient participation in the process of simultaneous recovery from mental illness and addiction ; it also networks the patient with community resources. Implementation of the clinical program involved addressing multiple clinical and organizational obstacles. The program has been operating more than 5 years and has treated more than 1,000 patients. As the token economy became an integral part of the ward routine, violence on the ward declined substantially, and patient participation in group activities increased significantly.
