FRANÇAIS : Les mycoses sont les infections les plus courantes chez les toxicomanes à la cocaïne, décédés du sida à Trinidad. Les germes proviennent de la fumée de crack ou de cannabis contaminée. Il en résulte diverses infections pulmonaires et des hépatosplénomégalies. La présence d'hépatosplénomégalie endémique dans certaines régions doit aider au diagnostic différentiel du sida. ENGLISH : Deep seated mycoses were the most common findings in cocaine addicts who died of AIDS in Trinidad, West Indies. Contaminated smoke either from crack, cocaine, marijuana or hemp was the suspected risk factor for the acquisition of Histoplasma and Aspergillus infection. Pulmonary infections were a feature in all cases in the study and massive hepato-splenomegaly were the most common pathological findings in histoplasmosis. In regions where histoplasma is endemic, the finding of massive hepatosplenomegaly in patients should alert the physicians to the possibility of the AIDS as a differential diagnosis.
