FRANÇAIS : Le cas des programmes d'échange de seringues de la ville de New York, depuis leurs origines clandestines en 1990 jusqu'aux premières années de fonctionnement officiel en 1992-1996, montre les problèmes rencontrés lors du passage de la clandestinité à la légalité. Les conflits sont fréquents entre les vieux activistes et les administrations qui établissent une certaine bureaucratisation : définition explicite des buts, procédures et limites, gestion, études, sensibilisation aux programmes méthadone et à la désintoxication. Conflit entre les volontaires de toujours et les nouveaux (et en faible nombre) salariés, conflit idéologique de pratiques (accès par pharmacie, distributeur, centres, réseaux d'usagers, équipe de rue). Les anciennes structures flexibles, spontanées et autonomes deviennent lourdes, ne s'adaptent plus aux besoins et demandes des usagers de drogues injectables ; mais l'officialisation permet une sécurité financière et donc un meilleur approvisionnement en seringues. Les différentes approches et stratégies nécessitent en fait d'être prise en compte pour augmenter l'efficacité du programme. ENGLISH : The initial period in the establishement of syringe exchange projects is often characterized by overt conflict between community AIDS activists, and public officials and political leaders. Professionals concerned with legitimating the new institutions of syringe exchange may sometimes neglect aspects of their everyday logistics and social organization, obscuring the important choices which have to be made to carry these initiatives forward. The advantages and disadvantages of each particular approach ("community-based", pharmacies, vending machines, users networks) make it likely that maximum effectiveness will be achieved through a combination of every possible form of needle distribution, and respond to diverse needs and target different populations. The case is here examined in the light of the experience of the SEPs in New York City, from their clandestine origins in 1990 through their first years of official functioning in 1992-1996.
